The biggest event of its kind by far, the reach and positive impact of Congress on patient care is felt long after the closing plenary – through the relationships it helps to build.

In September 2023, we
worked with Microsoft to produce a white paper, Digital Pathology and Integrated Care Systems, hoping to motivate advances that will reduce costs, increase efficiency and improve patient care. In January 2024, we worked with AstraZeneca to publish Time to Test – a paper on how to create the capacity the NHS needs for the future of cancer testing (see p.14).

THE WORKFORCE IN NUMBERS
Last year the IBMS published the Long Term Biomedical Scientist Workforce Plan. It presents a bold strategy for the UK to develop the biomedical scientist workforce to operate at the highest levels of practice.
Following are some of the striking statistics from the document.

Biomedical scientists are one of the longest established regulated healthcare professions, having been regulated by statute since 1960.

They also form the largest professional group within healthcare science (40%) and account for more than 27,000 of the Health and Care Professions Council (HCPC) registrants.
Biomedical scientists are the largest staff group in UK pathology labs at approximately 14,000.
UP TO 360,000 STAFF SHORT
In 2017, Health Education England predicted that if demand-led growth for healthcare remained constant, 190,000 new healthcare posts would be needed by 2027. Fast-forward to 2023 and the NHS England Long Term Workforce Plan predicts a shortfall of between 260,000 and 360,000 staff by 2036/37.

Only 3% of histopathology departments that responded to a survey said they had enough staff to meet clinical demand. The Royal College of Pathologists (RCPath) survey also revealed that a quarter of all histopathologists are aged 55 or over

Cancer Research UK estimates that, with a growing and ageing population, the projected increase in the number of cancer cases will be more than 40% above current levels – to about 514,000 new cases per year in 2035. Of the departments who responded to the RCPath survey questions on costs, around £9.8m a year is spent on locums, covering 77 posts at an annual average of £127,000 each.
If this figure was extrapolated to cover the UK, it would equate to around £17m.

The IBMS issues more than 1000 Certificates of Competence each year to biomedical science graduates from IBMS Accredited biomedical science degrees who have completed their pre-registration training in an IBMS-approved training laboratory.
HISTOPATHOLOGY REPORTING
Training biomedical scientists and clinical scientists to undertake selected histopathology reporting through IBMS qualifications is financially highly efficient.
£2000 The examination costs for a scientist to complete the four-year training programme to report the full spectrum of gastrointestinal or gynaecological samples is less than £2000
£1000 For a scientist to qualify to report bowel and cervical screening biopsies alone it takes approximately eighteen months with examination costs of less than £1000.
INTRODUCING THE COMPANY MEMBERS
Who are the Company Members and what is the aim of the scheme? We give an outline of their role and responsibilities.
In 1992, the IBMS launched the Company Membership scheme and the Company Members Liaison Group, which comprised Company Members and Institute Officers. The latter held its first meeting in 1992, which provided an excellent forum to discuss issues at the Liverpool Triennial and work with exhibitors to transform the event.
Fast-forward to the present and the scheme connects a members’ network of over 20,000 biomedical staff worldwide to many of the leading organisations in biomedical science, covering disciplines across the sector.
Delivering success
One of the Institute’s central aims for the scheme is to ensure that members are up to date with the latest developments in technology and advances in scientific techniques.
Working with the Company Members also enables the delivery of the highly successful annual Congress, while the companies also support local IBMS regional and branch scientific events and recognise members’ achievements through the award of the Company Members Prize, which is presented to the IBMS member who achieves the highest mark in each specialist

Essential for the future
The IBMS Company Members Committee (see box) works closely with IBMS committees and staff to help organise Congress and strengthen links between biomedical science companies and IBMS members.

DEVELOPMENTS IN POINT-OF-CAR E TESTING
Point-of-care testing (POCT) services are growing fast. To meet the challenge a suitable workforce with technical, operational and clinical skills is required. This would support the ongoing governance of services and ensure that tests of the appropriate quality are effectively deployed for a positive impact on existing and new patient pathways. Service
providers need to be agile, quick to respond to clinical need and have connections within the organisation that allow rapid evaluation and approval of the clinical and operational need for a new device or test.
To maintain current resources and deliver the workforce of the future, teams need to understand the mechanism of growing the establishment, funding streams and the process of

This can be achieved by in-year business cases that address a specific challenge or form the part of a larger case that describes the projected need based on how developments in healthcare are dictating the support required.
Qualifications and roles
There is no specific national job profile for any POCT role. Individual job descriptions and person specifications need to be written for roles in POCT and there are some excellent examples in use. I feel there is a need to now recognise POCT as a standalone specialty within biomedical science. It is unlikely that specific degrees will be tailored to POCT, but the existing biomedical science degrees would do well to incorporate an increasing level of POCT knowledge.
There are supplementary studies that graduates can undertake, including the IBMS Certificate of Expert Practice (CEP) in Point of Care. However, it is difficult to formalise the requirements for additional qualifications, knowledge and experience in current job descriptions. Many graduating biomedical scientists would not have been exposed to these services and will directly enter a recognised specialism of biomedical science.
This is one problem with recruiting to posts in POCT unless those scientists rotate through the POCT service as part of their local training.
The IBMS is reviewing the biomedical sciences portfolios, which may deliver a boost to the future workforce.
Innovations in technology
Techniques and methodologies in use across the rapid diagnostics landscape have evolved quickly in recent years with a surge in qualitative and quantitative lateral flow devices to molecular diagnostics. Sensor technology employed in analytical devices or those worn by patients have become more precise and less susceptible to interfering substances. Miniaturisation of analytics continues, with more microfluidic solutions coming to market, covering an impressive repertoire of tests. The multi-analyte device is becoming ever more popular, using the same pre-analytical and analytical processes for a range of tests, reducing the
different devices required to achieve the same panel of tests. Multiplex testing is proving popular, supporting clinical suspicion, and reducing time to clinical decision by analysing a wide range of targets in the same testing phase. Computer-assisted learning and artificial intelligence continue to make an impact on healthcare. Large datasets are being used to develop new algorithms to drive clinical decision support software, improving efficiency and standardising care. They remain essential to bring us a step closer to precision and personalised medicine, although care decisions must not be taken out of the qualified clinician’s hands.
Network maturation
The pathology networks set up through NHS England and NHS Improvement continue to mature, sights firmly set on
removing unwarranted variation in service delivery through the GIRFT (Get It Right First Time) agenda, collaborative working and improved consolidation.
The delivery of the NHS Long Term Plan depends, in part, on effective primary care networks, urgent treatment and diagnostic centres, same-day emergency care and expansion of virtual wards and hospital-at-home activities.
The creation of optimisation groups has led to an improved understanding of which test should be offered where and to whom, to have the maximum benefit on healthcare delivery. Community diagnostics centres are being designed to address backlogs in imaging and associated diagnostics and provide improved access for patients, avoiding hospital attendance. Improved diagnostic capabilities of urgent treatment centres is also supporting emergency services by managing specific cases away from emergency departments
Community, high street and at-home diagnostics
Diagnostics are more accessible than ever. There is certainly plenty of evidence to suggest that patients cared for in their own homes have faster recovery times and less risk than those recovering in hospital, if well enough to do so. Where necessary,

patients can now be sent home with diagnostic devices and tablets that allow for results to be submitted for review by a supervising clinician. The high street has also become a convenient source of diagnostic testing and consultations, with pharmacies keen to improve the range of services they can offer the public. A wide range of tests have now made their way into the public domain and in recent times it has become possible to order tests online or purchase in the supermarket, even the fuel station. The validity of these tests and the governance surrounding their provision is unknown to the author and cannot be commented on further. However, they are becoming more popular among a curious public who are taking more control over their own health and wellbeing.
Championing diagnostics, networking and events
I am a huge advocate for rapid diagnostics, if provided with excellent governance and patient safety remains the priority.
I’ve been providing POCT events twice a year since 2018 through
Thornhill Healthcare Events Ltd, along with consultancy and the POCTInnovators.com website, which champions the use of medical devices across multiple healthcare delivery points.
These events provide opportunities to share best practice, experience and knowledge. To ensure the events are accessible to the widest audience they are often offered free to the NHS, and from 2024 they are offered free to those in academia.
Although I work full-time in the NHS as well as providing events and consultancy, I am also a member of a BIVDA steering group, a tutor for the IBMS Certificate of Expert Practice in POCT, scientific advisory committee member to national and international commercial companies.
I encourage everyone to regularly attend these types of events, ensure your continuing professional development, and contribute to shared learning where you can. Collaborating and sharing is key to success.

IN FOCUS: DIGITAL PATHOLOGY
We look at the study that led to the government approving the use of digital pathology for cancer screening.
Following the Government and NHS England backing the UK National Screening Committee recommendation on digital pathology, there is likely to be an increase in the uptake and use of the technology.
Also, earlier this year, Minister of Health and Secondary Care Andrew Stephenson confirmed that NHS England is investing in a digital learning platform to
support the take-up of digital pathology technology in the health service.
The use of digital pathology in cancer screening samples has been embargoed until now, because there was deemed to be insufficient data to support its use in this setting.
Here we look at the National Institute for Health and Care Research (NIHR)funded study that led to the approval of digital pathology use
to help speed up analysis of cancer screening samples.
The research was carried out by University Hospitals Coventry and Warwickshire (UHCW) NHS Trust and The University of Warwick’s Clinical Trials Unit.

It was a huge task to do, but the data we produced was vital to demonstrate this technology is safe in the hands of our pathologists.’

The study’s aim was to demonstrate equivalence in pathologists’ reporting of a sample when using digital pathology in comparison to light microscopy, the current
A total of cases ( breast, GI, skin, renal) were included in the study. This included breast and bowel cancer screening samples. Cases were examined by four pathologists using both digital pathology and light microscopy. Reports were compared for clinical management concordance (CMC) – identical diagnoses plus differences that do not affect patient management. Percentage CMCs were computed using logistic regression models with crossed random-effects terms for case and pathologist. The obtained percentage CMCs were referenced to .% calculated from previous studies. For all cases, light microscopy versus digital pathology comparisons showed the CMC rates were .%.
standard. Sixteen pathologists took part in the study, four each in four specialities: skin, breast, GI and renal. The pathologists reported on each anonymous sample within their speciality using both digital pathology and light microscopy. In total, 2024 samples were used across all four specialities, and 16,192 reports were created during the study (see box, left, for more details).
The study was funded by the NIHR, Health Technology Assessment Programme and supported by the University of Warwick’s Clinical Trials Unit.

Ensure testing is organised according to the needs of patients and multidisciplinary teams, delivering commonly used or established tests closer to patients and at the quality and scale required.

Enable providers to deliver
When centralised testing services are unable to provide a timely, high-quality service, other providers should be supported to lend additional capacity, providing the right test at the right time.

Improve data
The publication of a genomics informatics implementation plan is a vital step towards improving quality and unlocking capacity.

Develop a cancer testing accountability framework
Quality measures should be established, which all NHS cancer testing providers adhere to, with comparable data published on performance.

Invest in the fundamentals
Investing in the fundamentals of infrastructure and logistics through increased capital funding would enable services to deliver high-quality tests with rapid turnaround times and support the expansion of services fit for the future.
